Chronic Care Management
Chronic care management is beneficial for patients in terms of ongoing health and wellness support, increased access to appropriate medical resources, enhanced communication with members of their care team, reduction in emergency room visits and hospitalization or readmissions, and increased engagement in their own healthcare.
Hormone Therapy and Weight Loss
With aging and weight gain, both men and women can develop an imbalance in hormone production. While it most commonly occurs in your 40s and 50s, a hormonal imbalance can develop earlier. Symptoms may appear subtle at first, and you may blame it on stress or aging, but before you know it, you can find yourself feeling tired, out of sorts and gaining more weight. Our experts explain how a hormonal imbalance can impact weight gain, and how hormone replacement therapy can help you lose weight and restore your vitality.

Love Dr 'G'ulbis. Very understanding and knowledgeable. Actually, takes time to listen before deciding what's best for you. Thankful there are still, down-to-earth, caring doctors. I drove all the way to Tomball when he moved, but am so glad he's back in Livingston, a much closer location for me. I'd recommend him and have recommended him to family and friends.

Dr. Gulbis has always been so amazing. He listens to his patients thoroughly and utilizes his knowledge and experience to help get you on the right road to recovery quickly. I have moved out of town but am considering coming home for visits just to keep seeing this wonderful doc.

Dr. Gulbis is the only doctor I would ever recommend. Usually I say they are all about equal but not Dr. G. He sets the bar extremely high with every aspect of his practice. He makes you feel like you are important to him on a personal level because you are. He is the ONLY doctor that actually took the time to listen to my wife and was able to find the correct diagnosis. He is the only doctor that takes the time to listen to everything you say. Yeah, you may wait a bit when your there. That’s only because he is giving his full attention to another patient. And when he comes in with that big ole smile, he will sit down and listen to you because you will have his full attention. We have been patients of his for several years. When he left Livingston the first time we were living in Houston and my wife had been driving up there to see him. We were happy he had moved closer. Now that he’s back in Livingston we will both make the trip for our appointments because hands down he is the ONLY doctor I trust for me and my family
